Background: Patients prescribed with medication that treats mental health conditions benefit the most compared to those prescribed with other types of medication. However, they are also the most difficult to adhere. The development of mobile health (mHealth) applications (‘apps’) to help patients monitor their adherence is fast growing but with limited evidence on their efficacy. There is no evidence on the content of these apps for patients taking psychotropic medication. The aim of this study is to identify and evaluate the (...) aims and functioning of available apps that are aiming to help and educate patients to adhere to medication that treats mental health conditions. Method: Three platform descriptions (Apple, Google and Microsoft) were searched between October 2015 and February 2016. Included apps need to focus on adherence to medication that treats mental health conditions and use at least a reinforcement strategy. Descriptive information was extracted and apps evaluated on a number of assessment criteria using content analysis. Results: Sixteen apps were identified. All apps included self-monitoring properties like reminders and psycho-educational properties like mood logs. It was unclear how the latter were used or how adherence was measured. Major barriers to medication adherence like patients’ illness and medication beliefs and attitudes were not considered nor where information to patients about mediation side effects. Very few apps were tailored and none was developed based on established theories explaining the processes for successful medication adherence like cognitions and beliefs. Reported information on app development and validation was poor. Discussion: A variety of apps with different properties that tackle both intentional and unintentional non-adherence from a different perspective are identified. An evidence-based approach and co-creation with patients is needed. This will ensure that the apps increase the possibility to impact on non-adherence. Theories like social cognition models can be useful in ensuring that patients’ education, motivation, skills, beliefs and type of adherence are taken into consideration when developing the apps. Findings from this study can help clinicians and patients make informed choices and pursue policy-makers to integrate evidence when developing future apps. Quality-assurance tools are needed to ensure the apps are systematically evaluated. (shrink)

The practice of surgery requires consideration of a number of specific aspects of clinical medical ethics that are different from those most influential in other areas of medical care. The nature of surgical care alters the sense of responsibility that surgeons feel for their actions and also alters the relationship between surgeons and patients. Because surgical care requires patients to place such great trust in their surgeons, surgical informed consent must emphasize the importance of that trust. Surgeons must use innovative (...) means to solve individual patient problems even if the result is a novel operation. Surgical procedures may be altered due to the unexpected findings in the operating room and therefore surgeons must have considered how to respond in such situations. The future of surgical practice will inevitably lead to increasing ethical concerns in maintaining the ethical dimension of surgery, in allowing autonomy for trainees while maintaining patients’ safety, and in balancing surgical risks of prophylactic surgery with the genetic predisposition to develop cancer. While natural law theory plays an important role for Catholic moral theology, it is true that Orthodox ethics has not endeavoured to develop its own theory of natural law. This article demonstrates the existence of the concept of natural law in Eastern Orthodox theology and argues that the main reason for this neglect is Eastern Christianity's traditional focus on faith rather than reason. In addition, the author, based on biblical and patristic grounds, highlights the necessity for a balance between the (...) two to be found and provides a way in which Orthodox moral theology could develop a natural law theory, should it ever feel the need to do so, by first redefining its relationship with the science of philosophy, after so many years of disregard. Kyle Stanford has recently given substance to the problem of unconceived alternatives, which challenges the reliability of inference to the best explanation (IBE) in remote domains of nature. Conjoined with the view that IBE is the central inferential tool at our disposal in investigating these domains, the problem of unconceived alternatives leads to scientific anti-realism. We argue that, at least within the biological community, scientists are now and have long been aware of the dangers of IBE. We re-analyse the nineteenth-century (...) study of inheritance and development (Stanford’s case study) and extend it into the twentieth century, focusing in particular on both classical Mendelian genetics and the studies by Avery et al. on the chemical nature of the hereditary substance. Our extended case studies show the preference of the biological community for a different methodological standard: the vera causa ideal, which requires that purported causes be shown on non-explanatory grounds to exist and be competent to produce their effects. The claim that consciousness constitutively involves self-consciousness has a long philosophical history, and has received renewed support in recent years. My aim in this paper is to argue that this surprisingly enduring idea is misleading at best, and insufficiently supported at worst. I start by offering an elucidatory account of consciousness, and outlining a number of foundational claims that plausibly follow from it. I subsequently distinguish two notions of self-consciousness: consciousness of oneself and consciousness of one’s experience. While “self-consciousness” is (...) often taken to refer to the former notion, the most common variant of the constitutive claim, on which I focus here, targets the latter. This claim can be further interpreted in two ways: on a deflationary reading, it falls within the scope of foundational claims about consciousness, while on an inflationary reading, it points to determinate aspects of phenomenology that are not acknowledged by the foundational claims as being aspects of all conscious mental states. I argue that the deflationary reading of the constitutive claim is plausible, but should be formulated without using a term as polysemous and suggestive as “self-consciousness”; by contrast, the inflationary reading is not adequately supported, and ultimately rests on contentious intuitions about phenomenology. I conclude that we should abandon the idea that self-consciousness is constitutive of consciousness. (shrink)

Statues of gods in Greek culture had lives, both metaphorically and literally. The statues of gods had complex ritual lives. They had biographies (bioi); they travelled; they were subject to peripeties (destruction, repairs, re-dedication); and they suffered violence. Although they were not an indispensable element of worship, the images psychologically prepared the worshippers to address the divinity, and this was an important factor in the efforts of worshippers to communicate with the gods. Through the arousal of emotions they provoked actions (...) that were beyond or contrary to human intentions; in this sense, they had agency. Finally, under certain conditions they were perceived as fulfilling vital functions. Since Greek gods could not be in two places at the same time, they could not continually inhabit their statues. But they manifested their power through the statue during an epiphany. A statue was a receptacle (ankeion) of a god and medium of epiphany. (shrink)

The ambition of this article is to propose a way of visualizing the Anthropocene dialectically. As suggested by the Dutch atmospheric chemist Paul Crutzen and the professor of biology Eugene F. Stoermer, the term Anthropocene refers to a historical period in which humankind has turned into a geological force that transforms the natural environment in such a way that it is hard to distinguish between the human and the natural world. Crutzen and Stoermer explain that the Anthropocene has begun after (...) the Holocene, the geological epoch that followed the last ice age and lasted until the industrial revolution. Drawing on a number of figures such as the “tenfold” increase in urbanisation, the extreme transformation of land surface by human action, the use of more than 50% of all accessible fresh water by humans, and the massive increase in greenhouse emissions, Crutzen and Stoermer conclude that the term Anthropocene describes aptly mankind's influence on ecological and geological cycles (Crutzen & Stoermer, 20... This article explores the consecutive modifications that phenomenology underwent in the works of Heidegger and Levinas. In particular, it discusses their importance for contemporary attempts to expand — and transcend — phenomenology in philosophy and the social sciences. Heidegger and Levinas responded to the problem of subjectivity — and intersubjectivity — in diametrically opposed ways and consequently the exposition of their thoughts involves focusing on conceptual dichotomies like finitude and infinity, time and eternity. Ultimately, it is argued that the very (...) conceptions of the human being that the two thinkers furnish under the guise of Dasein and Autrui reveal the impasses associated with western metaphysics and pave the path for the development of more adequate theorizations of subjectivity, radical otherness and communal being. There is converging evidence that high doses of hallucinogenic drugs can produce significant alterations of self-experience, described as the dissolution of the sense of self and the loss of boundaries between self and world. This article discusses the relevance of this phenomenon, known as “drug-induced ego dissolution (DIED)”, for cognitive neuroscience, psychology and philosophy of mind. Data from self-report questionnaires suggest that three neuropharmacological classes of drugs can induce ego dissolution: classical psychedelics, dissociative anesthetics and agonists of the kappa opioid (...) receptor (KOR). While these substances act on different neurotransmitter receptors, they all produce strong subjective effects that can be compared to the symptoms of acute psychosis, including ego dissolution. It has been suggested that neuroimaging of DIED can indirectly shed light on the neural correlates of the self. While this line of inquiry is promising, its results must be interpreted with caution. First, neural correlates of ego dissolution might reveal the necessary neurophysiological conditions for the maintenance of the sense of self, but it is more doubtful that this method can reveal its minimally sufficient conditions. Second, it is necessary to define the relevant notion of self at play in the phenomenon of DIED. This article suggests that DIED consists in the disruption of subpersonal processes underlying the “minimal” or “embodied” self, i.e., the basic experience of being a self rooted in multimodal integration of self-related stimuli. This hypothesis is consistent with Bayesian models of phenomenal selfhood, according to which the subjective structure of conscious experience ultimately results from the optimization of predictions in perception and action. Finally, it is argued that DIED is also of particular interest for philosophy of mind. On the one hand, it challenges theories according to which consciousness always involves self-awareness. On the other hand, it suggests that ordinary conscious experience might involve a minimal kind of self-awareness rooted in multisensory processing, which is what appears to fade away during DIED. Raphael examines the moral philosophy of Adam Smith (1723-90), best known for his famous work on economics, The Wealth of Nations, and shows that his thought still has much to offer philosophers today. Raphael gives particular attention to Smith's original theory of conscience, with its emphasis on the role of 'sympathy' (shared feelings).

This article is based on the assumption that core concepts of National Socialism—different from Marxism—turn not on economic, but on moral concepts, or categories heavily related to such concepts as honour, loyalty, decency and comradeship. The article investigates National Socialism from the standpoint of moral judgments, and turns this investigation into part of a moral history. It further is concerned with the continuing impact of National Socialism beyond the military, political and ideological defeat of 1945; the moral historical approach can (...) show how this influence continues at a level hitherto relatively unexamined. The moral history of National Socialism involves more than a shift of perspective. It opens up a specific, barely explored, field of research. This draws upon a variety of sources, especially films (Hotel Sacher—1939 and Downfall—2004 are discussed). The examination is directed to the concept of loyalty within this framework; it is a concept of virtue, which is moreover suited to illuminate some aspects of the functioning of Nazi morality, and its subsequent influence. Many authors argue that conscious experience involves a sense of self or self-consciousness. According to the strongest version of this claim, there can be no selfless states of consciousness, namely states of consciousness that lack self-consciousness altogether. Disagreements about this claim are likely to remain merely verbal as long as the target notion of self-consciousness is not adequately specified. After distinguishing six notions of self-consciousness commonly discussed in the literature, I argue that none of the corresponding features is necessary for (...) consciousness, because there are states of consciousness in which each of them is plausibly missing. Such states can be said to be at least partially selfless, since they lack at least one of the ways in which one could be self-conscious. Furthermore, I argue that there is also preliminary empirical evidence that some states of consciousness lack all of these six putative forms of self-consciousness. Such states might be totally selfless, insofar as they lack all the ways in which one could be self-conscious. I conclude by addressing four objections to the possibility and reportability of totally selfless states of consciousness. (shrink)

Clinical trials emerged in rapid succession as the COVID-19 pandemic created an unprecedented need for life-saving therapies. Fair and equitable subject selection in clinical trials offering investigational therapies ought to be an urgent moral concern. Subject selection determines the distribution of risks and benefits, and impacts the applicability of the study results for the larger population. While Research Ethics Committees monitor fair subject selection within each trial, no standard oversight exists for subject selection across multiple trials for the same disease. (...) Drawing on the experience of multiple clinical trials at a single academic medical centre in the USA, we posit that concurrent COVID-19 trials are liable to unfair and inequitable subject selection on account of scientific uncertainty, lack of transparency, scarcity and, lastly, structural barriers to equity compounded by implicit bias. To address the critical gap in the current literature and international regulation, we propose new ethical guidelines for research design and conduct that bolsters fair and equitable subject selection. Although the proposed guidelines are tailored to the research design and protocol of concurrent trials in the COVID-19 pandemic, they may have broader relevance to single COVID-19 trials. (shrink)

Deep learning algorithms are rapidly changing the way in which audiovisual media can be produced. Synthetic audiovisual media generated with deep learning—often subsumed colloquially under the label “deepfakes”—have a number of impressive characteristics; they are increasingly trivial to produce, and can be indistinguishable from real sounds and images recorded with a sensor. Much attention has been dedicated to ethical concerns raised by this technological development. Here, I focus instead on a set of issues related to the notion of synthetic audiovisual (...) media, its place within a broader taxonomy of audiovisual media, and how deep learning techniques differ from more traditional approaches to media synthesis. After reviewing important etiological features of deep learning pipelines for media manipulation and generation, I argue that “deepfakes” and related synthetic media produced with such pipelines do not merely offer incremental improvements over previous methods, but challenge traditional taxonomical distinctions, and pave the way for genuinely novel kinds of audiovisual media. In recent years, the scientific study of meditation and psychedelic drugs has seen remarkable developments. The increased focus on meditation in cognitive neuroscience has led to a cross-cultural classification of standard meditation styles validated by functional and structural neuroanatomical data. Meanwhile, the renaissance of psychedelic research has shed light on the neurophysiology of altered states of consciousness induced by classical psychedelics, such as psilocybin and LSD, whose effects are mainly mediated by agonism of serotonin receptors. Few attempts have been made (...) at bridging these two domains of inquiry, despite intriguing evidence of overlap between the phenomenology and neurophysiology of meditation practice and psychedelic states. In particular, many contemplative traditions explicitly aim at dissolving the sense of self by eliciting altered states of consciousness through meditation, while classical psychedelics are known to produce significant disruptions of self-consciousness, a phenomenon known as drug-induced ego dissolution. In this article, we discuss available evidence regarding convergences and differences between phenomenological and neurophysiological data on meditation practice and psychedelic drug-induced states, with a particular emphasis on alterations of self-experience. While both meditation and psychedelics may disrupt self-consciousness and underlying neural processes, we emphasize that neither meditation nor psychedelic states can be conceived as simple, uniform categories. Moreover, we suggest that there are important phenomenological differences even between conscious states described as experiences of self-loss. As a result, we propose that self-consciousness may be best construed as a multidimensional construct, and that “self-loss,” far from being an unequivocal phenomenon, can take several forms. Indeed, various aspects of self-consciousness, including narrative aspects linked to autobiographical memory, self-related thoughts and mental time travel, and embodied aspects rooted in multisensory processes, may be differently affected by psychedelics and meditation practices. Finally, we consider long-term outcomes of experiences of self-loss induced by meditation and psychedelics on individual traits and prosocial behavior. We call for caution regarding the problematic conflation of temporary states of self-loss with “selflessness” as a behavioral or social trait, although there is preliminary evidence that correlations between short-term experiences of self-loss and long-term trait alterations may exist. In this new and enlarged edition of a standard introduction to moral philosophy, Raphael shows in clear and simple language the connections between abstract ethics and practical problems in law, government, medicine, and the social sciences in general. Moral Philosophy deals with six main areas. First, it looks at the two opposed traditions of naturalism and rationalism, and considers more recent discussion in terms of logic and language. It is widely assumed that ordinary conscious experience involves some form of sense of self or consciousness of oneself. Moreover, this claim is often restricted to a ‘thin’ or ‘minimal’ notion of self-consciousness, or even ‘the simplest form of self-consciousness’, as opposed to more sophisticated forms of self-consciousness which are not deemed ubiquitous in ordinary experience. These formulations suggest that self-consciousness comes in degrees, and that individual subjects may differ with respect to the degree of self-consciousness they exhibit at a (...) given time. In this article, I critically examine this assumption. I consider what the claim that self-consciousness comes in degrees may mean, raise some challenges against the different versions of the claim, and conclude that none of them is both coherent and particularly plausible. The research is developed with the use of deductive and descriptive analytical methods used to obtain information on doctrine and jurisprudence and to establish and describe specific situations in the field of Ecuadorian Medical Law respectively. The main objective is to provide scientific and doctrinal tools to understand guilt in cases of omissive conduct in Ecuadorian medical practice, as well as its comparison with other countries such as Colombia, Mexico, Cuba and Argentina, the results of the research show that the (...) doctrine in the relationship to the categories of Omission, Guilt, Guarantor Position and Permitted Risk is broad and is based on scientific-doctrinal advances of the Normative Conception of Guilt comprised between 1995 and 2022. To establish useful criteria and conclusions, the discussion focuses on defining what is considered “guilty” in the context of Medical Law, highlighting the importance of elements such as the objective duty, the harmful result and the analysis of the concomitant circumstances, emphasizing the importance of considering the context and development of the facts in cases of omissive conduct in medicine and not only the result. In relation to omissions in criminal law, a distinction is made between proper and improper and finally, it concludes by suggesting the modification of article 146 of the Organic Integral Criminal Code to guarantee due process and a effective judicial protection in matters related to health in Ecuador, specifically with regard to causality in cases of omissive conduct. This special issue is about something most of us might find very hard to conceive: states of consciousness in which self-consciousness is radically disrupted or altogether missing.

Die Zwischenkriegszeit des 20. Jahrhunderts war die Hochzeit der Kulturphilosophie. Ihre zentrale Forderung nach einer Trennung der Realwissenschaften in Naturund „Kulturwissenschaften“1 stand der Unternehmung zu einer Einheitswissenschaft des Wiener Kreises diametral entgegen. Auch Moritz Schlick sprach sich Zeit seines Lebens entschieden gegen die „mächtig emporgeblüht[e]“ Mode der „Kulturwissenschaften oder Geisteswissenschaften“ (Schlick, MSGA I/6, 535) aus. Dies geht beispielhaft aus seinem Vortrag über das Verhältnis von „Philosophie und Naturwissenschaft“ (Schlick, MSGA I/6, 521–545) hervor, in dem er ausführt, dass jene „Wissenschaften vom (...) Menschlichen“ schlechthin nicht über eigene Grundbegriffe verfügten und deswegen stets auf die „ausgezeichnete Stellung des naturwissenschaftlichen Denkens“ angewiesen (Schlick, MSGA I/6, 536–538) blieben, namentlich auf die Psychologie als die „Brücke zur Geschichte und zu den Geisteswissenschaften überhaupt“ (Schlick, MSGA I/6, 525f.). Insofern könnten sie für sich nicht den Anspruch erheben, eine vom naturwissenschaftlichen „Weltbild“ verschiedene „Weltanschauung“ spekulativ zu entwerfen (Schlick, MSGA I/6, 541f.). Die Gegenstände der Kulturwissenschaften, so zeigt sich Schlick auch später noch überzeugt, seien letztlich psychologisch zu ergründende Handlungen von Menschen und stünden daher „nicht im absolutem Gegensatze […] zu den Gegenständen der Naturwissenschaften“.
